Part 1:

__ Education Training Voucher Application
__ ETV Budget Form
__ ETV Student Education Agreement
__ ETV Financial Aid Release Form
__ Copy of high school diploma or GED
__ Copy of class schedule/enrollment
__ Copy of college/trade school Grade Point Average (GPA)
__ Copy of financial aid package (financial aid received and financial need)
__ Copy of cost of tuition
__ Receipts
__ Budget Documentation/Estimates/Bills
__ Assessment Completed

Part 2, 3, and 4:

The Education and Training Voucher (ETV) is awarded each term for a maximum of $5000 for the fiscal year. A full time student may receive up to $2000 twice per fiscal year and a part time student may receive $1000 four times per fiscal year. To obtain and additional ETV amount, the ETV Re-Determination Application and Budget must be completed. This form cannot be completed until your first installment of ETV has been paid out and you have completed your courses you are currently enrolled in and have proof of completion. The application, budget, and additional documentation is required to receive further funding.

If the Educational Training Voucher will be used for other expenses also provide documentation to support need. For example but not limited to:
__ Rent/housing: copy of lease agreement
__ Computer (limit of $1,500 every 4 years): estimate from store you plan to buy it from
__ Transportation expense (limit of $2,500 for purchase of vehicle every 4 years): copy of payment stubs, car insurance bill, price estimate from the dealership you place to purchase vehicle from. If purchasing vehicle, the following must be provided: copy of drivers license, copy of insurance, inspection from licensed mechanic.
__ Daycare: written statement from daycare provider
An accredited school means:
__ The school is legally authorized within the State to provide a program of education beyond secondary education; AND
__ The school is a public, private or other nonprofit institution; AND
__ A nationally recognized accrediting agency or association accredits the school AND
__ The school provides an educational program where the institution awards a bachelor's degree or provides not less than a 2-year program that is acceptable for full credit toward such a degree,
__ The school provides an educational training program to prepare students for gainful employment in a recognized occupation;
